Job Description :
We are seeking a dedicated Case Manager/Community Health Worker to join our team. The ideal candidate will be passionate about working with vulnerable populations and will be responsible for helping clients access necessary services, providing ongoing support, and collaborating with other healthcare professionals to ensure coordinated care. You will serve as an advocate and educator, helping clients navigate healthcare systems, connect with resources, and work toward personal goals to support them on their recovery journey. 

Key Responsibilities :

• Assist clients in accessing medical, mental health, and social services
• Coordinate care with healthcare providers, social services, and community resources
• Conduct assessments to determine clients’ needs and develop individualized care plans
• Monitor and evaluate clients' progress, providing ongoing support and interventions
• Educate clients about health management, treatment options, and available resources
• Advocate for clients to ensure they receive the necessary care and services
• Maintain accurate records of client interactions and progress
• Collaborate with the clinical team to ensure quality care and adherence to treatment plans
• Facilitate support groups, workshops, or educational sessions for clients and their families
• Provide crisis intervention and emotional support as needed
• Must be willing to be onsite and conduct community outreach to clients 

Qualifications :

• High school diploma or equivalent; Bachelor's degree in a health-related field preferred
• Certification as a Community Health Worker (CHW) is a plus
• Minimum 1 year of experience in case management, social services, or community health
• Strong communication and interpersonal skills
• Ability to work with diverse populations, including individuals with mental health and substance use challenges
• Knowledge of community resources, healthcare systems, and social services
• Strong organizational skills and attention to detail
• Ability to work both independently and as part of a team
