Salary: $40,000-$45,000

HIRING BONUS OF $1000!

Primary Responsibilities  

 

  • Responsible for the provision of all intensive case management services required to maintain children with intensive medical, mental, emotional, and/or behavioral needs in a single placement to encourage safety and stability. 
  • To ensure safety, provides face-to-face contact at a frequency of two times per week during the first 3 months and then a minimum of one time per week thereafter based on needs of the child(ren). Visits will occur both in the foster home and in the Family Ark office or in another appropriate community setting. 
  • Participates in the ongoing assessment of the appropriateness of the foster care designation providing input and recommendations related to this designation. 
  • Provides intensive case management support to foster parents and youth on their caseload. 
  • Participates in and leads foster parent continuing training sessions. 
  • Provides independent living (IL) instruction as well as IL related activities for youth aged 16 and older as outlined in the Foster Care Master Contract. 
  • Reviews all safety plans in place with the youth and foster parents to ensure understanding and adherence in the home. 
  • Carries a caseload of approximately 8-12 children. Caseload size will vary depending on the needs/level of children being served.
  • Maintains all client case management/case records, including Treatment Plans, Monthly Reports, Case Notes, etc. 
  • Collaborates and documents client contacts, as well as contacts with referral sources, schools, foster parents, and other professionals involved in the care of each client on their caseload. 
  • Participates in agency-wide staff meetings. 
  • Assesses environmental factors in the foster home. 
  • Attends weekly staff meetings. 
  • Completes all required time reports, service documentation, and other required paperwork, according to programmatic and funding source requirements.
  • Complies with all Quality Improvement initiatives. 
  • Participates in 24-hour on-call rotation with the foster care program staff. 
  • Attends continuing education classes, seminars, and other professional training courses relating to family issues and professional development. 
  • Provides own transportation to assigned homes when company cars are not available. 
  • Complies with federal, state, and local laws concerning duty to warn and mandated statutes including mandatory reporting of child abuse and neglect. 

 

Minimum Requirements  

 

  • Bachelor’s degree in social work, psychology, sociology, or a directly-related human service field from an accredited college 
  • One year experience in social services or internship in social science field 
  • Foster Care experienced preferred 
  • Adhere to the Family Ark Professional Standards of Practice
