Position Summary
The Community Health Nurse Practitioner is responsible for health maintenance, disease prevention, diagnosis, and treatment of common episodic and chronic conditions. The community health nurse practitioner will focus on providing services to patients in underserved areas that need access to quality healthcare. The community health nurse practitioner will also provide services to special patient populations to include HIV, Hepatitis C and other communicable diseases. The Community Health Nurse Practitioner must be able to deliver HIV prevention services, including follow-up HIV testing, prescribing PrEP and PEP, HIV testing counseling and appropriate referral to community resources. The applicant must feel comfortable working with patients in the LGBTQIA+ community and foster a welcoming and inclusive environment. The community health nurse practitioner will serve as the HIV clinical consultant for the organization and provide routine clinical updates for staff. This position requires the provider to provide healthcare services in the community, outside of a typical clinic setting. This may include participating in health fairs and seeing patients at “pop up clinics” and on a mobile unit.
Essential Duties and Responsibilities
- Performs initial and ongoing assessments of patients' medical, physical and psychological status, including histories and physical examinations.
- Develops treatment plans individualized to the patient's needs.
- Records notes in the medical record.
- Writes orders for medications, treatments, tests, and provides direct patient care and education
- Provide health maintenance, chronic disease co-condition management and HIV specialty care for patients living with HIV infection/AIDS.
- Coordinate services for patients living with HIV/AIDS with the support of registered nurses, medical assistants, health educators, and case managers.
- Provide HIV pre-exposure prophylaxis (PrEP) to persons at risk for HIV infection and HIV post exposure prophylaxis (PEP) for persons with recent HIV exposure.
- Serve as the HIV clinical consult for all staff within the agency.
- Serve as the CQM (clinical quality management) committee lead for the Ryan White Program
- Provide routine updates for staff on HIV treatment, care and management
- Responds to organizational needs for leave coverage, office emergencies, chart completion, forms completion, peer review, lab review, and other needs in a cooperative fashion.
- Stays current in professional field, maintains licensure and follows principles of evidence- based medicine and clinical guidelines adopted by the organization.
- Performs all the essential duties of Adult Medicine staff provider
- Performs other related duties as assigned by management
Specific Functions
- Primary care/health maintenance health screenings in the community, patient education about screening results and need for follow up, referrals to the adult medicine/peds clinic to establish care with a primary care provider and "gaps in care" services. Offered on a rotating basis in our service area.
- HIV: Serve as the primary provider for all newly diagnosed, new to care and returning to care patients living with HIV. Serve as the clinical consult for staff with HIV related questions. Provide routine (quarterly) staff updates and in-service to all clinical staff. Serve as the provider for all DHEC PrEP referrals. Meet with all HIV positive patients annually for a HIV focused annual wellness visit
- Ryan White: Serve as the RW CQM committee lead. Hold monthly care conferences with MCMs and providers on select clients to ensure all client needs are met, identify barriers to care and increase retention and viral load suppression .
- Administration/Outreach: Meetings, leadership duties, charting f/u, health fairs

Qualifications
- Graduate of an accredited Nurse Practitioner program
- Must be a licensed Nurse Practitioner in the state of South Carolina
- Must be comfortable with the LGBTQ+ community and discussing sexual health
- Previous HIV/AIDS experience preferred
- Meets all the qualifications of the Nurse Practitioner staff position

- Specialized equipment, machines, or vehicles used: otoscope/ophthalmoscope ; EKG, inhalation nebulizer, microscope, etc.

Never shares data with colleagues without verifying the identity of the person requesting it and checking that the reasons for sharing are valid.
