We're seeking a compassionate and dedicated Population Health Case Manager to join our team and help improve the health and well-being of individuals in our community. This hybrid role offers the flexibility of remote work combined with in-person community engagement.

What You'll Do:

Connect with People: Build meaningful relationships with individuals in need, understanding their unique challenges and aspirations.
Empower and Educate: Provide guidance, education, and support to help individuals make informed decisions about their health and well-being.
Coordinate Care: Collaborate with healthcare providers, social workers, and other professionals to create comprehensive care plans.
Advocate for Change: Advocate for your clients' needs and connect them with essential community resources.
Track Progress: Monitor and measure outcomes to ensure the effectiveness of your interventions.
What You'll Need:

Bachelor's degree in nursing, social work, or a related field
Experience in case management, health, or behavioral health
Knowledge of social determinants of health, mental health, and substance abuse disorders
Strong interpersonal and communication skills
Ability to work independently and as part of a team
Valid driver's license and reliable transportation
